You must understand that it your responsibility to let us know if any pre certifications or in a referral is required for any office visits, inpatient or outpatient admissions as well as any surgeries.
For service you must authorize payment directly to NTCR for the medical and/or surgical benefits otherwise payable to you. You also must understand that you will be financial responsible for the charges not covered by your insurance.
